Veterans in Need Program Application

Submitted application can take up to 72 hours to complete.

* Any application found to have intentionally misleading or false information will disqualify submitter and Service Member (SM) from applying from future assistance with TVSV.

Do not state situation is urgent unless truly an emergency.

TVSV will make payment directly to payee, caseworker/organization or housing coordinator, the submitter will be responsible for notifying the service member.

All applications will require vetting and completion of all submitted applications.

Any additional information that is requested from the TVSV Executive Board will be made by email and the submitter will have an opportunity to resubmit the application within 24-48 hours.

Organization submitting the application.
Submitter’s Name.
Submitter’s daytime contact number.
Service Member name as appears on their ID.
Service Member daytime phone number.
SM current residence if it’s a hotel name of hotel.
Please identify what identification was used to vet the service member.
Did submitter verify SM military status?
Gross monthly income for ENTIRE HOUSEHOLD no exceptions
Identify where the SM is currently working.
If SM is not working is he/she currently working with a DVOP or employment specialist
If not please explain why not.
Enter specific amount being requested for
Is this a new request?
Has the SM previously applied for assistance from TVSV?
If so, when, and how much?
Identify what the specific need is being used for. COVID funds will be for limited time
Should the request require a bus ticket out of town, submitter must perform a verification from the Superior Court, click on link, type in the characters and search by name. This step is mandatory. Save this link to your favorites: Superior Court

The mission of TVSV is independence and self-sufficiency, this field must be able to include a plan moving forward after assistance is provided. Provide specific justification for the request, Help explain the following questions below:

  1. Define the problem
  2. Collect the data with all pertinent information
  3. Ask why the request is needed
  4. Determine which factors are the root cause
  5. Identify the correction action that will help the problem from recurring
  6. Implement the solution that will take place should the application be approved

REQUIRED

List 3 Organizational Agencies where Veteran sought assistance prior to TVSV, please call Be Connected at 1-866-49-8387 for a list of available agencies within the Tucson Community. All fields below must be completed.

TVSV is a last resort and application must demonstrate due diligence in contacting other organizations/resources before applying with TVSV by either the case manager or Veteran. Must list 3 organizations, person spoken to, phone number, AND RESPONSE! If Veteran cannot remember who they contacted, explain mitigating circumstances. “Left message” will not be accepted.
